You earn less, yet the premium grew — retirement is the day your status changes
Health insurance has three statuses: employee member, regional member, and dependent. The 2026 rate of 7.19% is the same for all, but what it is applied to differs. Employees are charged on salary alone and the employer pays half. Regional members pay it all themselves, with a property premium added to the income premium (pension income counts at only half). Dependents pay nothing but must pass all three tests (income of ₩20M a year or less, no business income, and a property tax base of ₩540M or less), and the income test is applied to both spouses.
Conditions used in the calculation
- 2026 health insurance rate
- 7.19% (same for employee and regional members)
- Employee example
- salary ₩3M, your share 3.595% (employer pays half)
- Regional member example
- National Pension ₩2M a month (₩24M a year) · no other income · home with a ₩220M property tax base · no loan
- Income premium formula
- public pension and earned income count at 50% (Rule Art. 44) → ₩2M × 50% = ₩1M × 7.19%
- Property premium formula
- property tax base − ₩100M basic deduction → Table 4 points × ₩211.5
Results
- Employee's share (₩3M × 3.595%)
- ₩107,850
- Regional income premium (₩2M pension × 50% × 7.19%)
- ₩71,900
- Regional property premium (₩220M − ₩100M = ₩120M → 490 points × ₩211.5)
- ₩103,635
- Regional total (₩71,900 + ₩103,635)
- ₩175,535 (₩67,685 more than as an employee)
- Why the dependent route is closed
- ₩24M a year in pension is over the ₩20M income test (pension counts in full for dependents)
- Dependent income test
- ₩20M a year or less + no business income (up to ₩5M if unregistered · any housing rental income closes the exception)
- Dependent property test
- property tax base ₩540M or less (up to ₩900M if annual income is ₩10M or less)
- Premium on cars
- abolished from the February 2024 bill (after 35 years, since 1989)
At a glance
The property premium starts after a ₩100M deduction; a dependent is disqualified above ₩540M (₩900M if annual income is ₩10M or less).
- Property premium basic deduction₩100M
- Dependent property test₩540M
- Maximum when annual income is ₩10M or less₩900M
Why it works this way
You worked 30 years and retired. With the salary gone, your outgoings should shrink too. But a few months later a health insurance bill arrives, larger than when you were employed. Income vanished and the premium grew. Your first thought is that it must be a mistake, but the calculation is right. Your status changed. Health insurance has three statuses, each with its own formula. Which one you stand in sets the amount, and retirement is the day the status changes.
The three statuses are employee member, regional member, and dependent. An employee is charged on salary alone and the employer pays half. A regional member is charged on income and property and pays in full. A dependent pays nothing, but the door in is narrow. One person moves through all three over a lifetime: employee when hired, regional when retired, dependent when the tests are met. Four steps: ① how do employee and regional differ, ② how is a regional member's property premium calculated, ③ how narrow is the dependent's door, ④ what can you prepare before retiring.
Step one. The 2026 health insurance rate is 7.19%, the same for employee and regional members. What differs is what the rate is applied to and who shares it. For an employee it is applied to monthly pay (the salary) and split with the employer, so your share is 3.595%. On ₩3M a month, ₩107,850 leaves your pay. Property does not matter. Own a home and an employee's premium is still charged on salary alone.
A regional member is different. A property premium is added to the income premium (National Health Insurance Act Art. 69(5)), and with no employer to share it, it is all yours. But income is counted differently from salary. Public pensions such as the National Pension and earned income count at only half, and interest and dividends taxed separately are left out entirely if they total ₩10M a year or less (Enforcement Rule Art. 44, amended 2024-05-13 · current Rule effective 2026-08-11). A retiree on a ₩2M-a-month National Pension is counted as having ₩1M of income, so the income premium is ₩1M × 7.19% = ₩71,900 a month. That is actually less than the ₩107,850 paid while employed. Yet the bill grows, because the property that was hidden behind the salary enters the calculation. One home with a ₩220M property tax base adds a property premium of ₩103,635 (worked out in step two), for a total of ₩175,535. That is ₩67,685 more than as an employee. The home you spent 30 years paying for becomes, for the first time in 30 years, the base of a premium.
Same 7.19% rate. The income premium on a ₩2M pension is lower than as an employee, but the property premium on a home with a ₩220M tax base pushes the total higher. Long-term care insurance is charged separately on both sides, and the billed amount may differ by a few won from rounding.
- Employee · ₩3M salary · 3.595% share₩107,850
- Regional income premium · 50% of ₩2M pension × 7.19%₩71,900
- Regional property premium · ₩220M tax base · 490 points₩103,635
- Regional total₩175,535
Step two: the property premium. Take the property tax base, subtract a ₩100M basic deduction, convert the remainder into points on a bracket table, and multiply each point by ₩211.5. That is the method in Table 4 of the Enforcement Decree (Table 4 amended 2024-05-07). With a ₩220M tax base, subtracting ₩100M leaves ₩120M, which falls in grade 20 (over ₩119M up to ₩133M) for 490 points: 490 × ₩211.5 = ₩103,635. Owning property does not trigger a premium by itself. Up to a ₩100M tax base there is no property premium. The figure used is the property tax base printed on your property tax bill, not market value, and it is usually far below market value.

Cars were charged for 35 years from 1989 and dropped from the February 2024 bill. However many cars you own, they have no bearing on health insurance.
What about cars? They are no longer in the calculation. From 1989, for 35 years, cars carried a premium too, but it was abolished from the February 2024 bill. However many cars you own, they have no bearing on your health insurance. If you have been worrying, on the old rules, that a car would raise your premium, that worry no longer applies.
Step three: the dependent. This is the status that pays nothing at all, a seat attached to a family member who is an employee. But the door has three layers and you must pass all three. ① Income: over ₩20M a year and you are out, and public pensions count in full here, not at half. A ₩2M-a-month National Pension is ₩24M a year, so it stops at this first door. ② Type of income, the layer most often missed. Even under ₩20M, any business income disqualifies you. Only if you have no business registration is up to ₩5M tolerated, and any housing rental income closes even that exception. So 'just stay under ₩20M' is only half right. ③ Property: if the property tax base exceeds ₩540M, you survive only up to ₩900M and only if annual income is ₩10M or less. Above that, you are out. And the income test is not applied to spouses separately. If one spouse fails the income test, neither can be a dependent. Your own income can be ₩0 and you can still be disqualified by your spouse's business income. Nor is passing once the end of it: the Service links income and property records and re-checks eligibility. For someone who ran the numbers in advance, that notice is no surprise.
Step four: what you can prepare before retiring. ① Check your property tax base now. The property premium bracket and the dependent property test both come from this one figure, which is on the July and September property tax bills. ② Sketch the types of income you will have after retiring. The same amount gives different results by type, and rental income in particular, even a single won, shuts the dependent's door. ③ There is a scheme called voluntary continued membership: for a period after leaving a job, it lets you keep paying at roughly the level you paid as an employee, a buffer for when the regional premium would be larger. Eligibility and duration need confirming with the Service, and there is an application deadline you cannot recover once missed, so as retirement approaches, ask the National Health Insurance Service first. Your premium after retirement is set by the income and property structure you built before it, not on the day you retire.
One table to sum up. Employee: salary × 7.19%, half paid by the employer, property not counted. Regional: income (pension and earned income at half) × 7.19%, paid entirely by you, plus the property-point premium. Dependent: ₩0, but three steps (₩20M income, no business income, property tax base ₩540M). Three boxes are misread most often. An employee's premium does not rise however much property they own. A regional member is charged on property alone even with no income. A dependent's income test is applied to both spouses. For reference, an employee with very large income outside salary faces a separate additional premium.
Two things to do now. ① Find the tax base on your property tax bill and compare it with ₩100M and ₩540M. ② If retirement is in sight, confirm eligibility and the deadline for voluntary continued membership with the Service in advance. Today covered only the skeleton of the three statuses: the property point brackets, the detailed income assessment, and regional premium reductions differ by case, and the Service's simulator and consultation are the most accurate. The bill arrives after you retire, but the amount is decided before. Only those who look ahead get to choose.

A regional member's monthly premium is the sum of the income-based premium and the property-based premium (property premium points). An employee's premium is monthly pay multiplied by the premium rate.
What this means
Employees are charged on salary alone; regional members on income and property. When retirement changes your status, property that was hidden behind the salary enters the calculation.

The 2026 premium rate is 719/10000 (7.19%) and the amount per property premium point is ₩211.5. For employees, the member and the employer each bear half.
What this means
The rate is the same for employee and regional members, and an employee's own share is 3.595%, or ₩107,850 on ₩3M.

A regional member's monthly income is assessed from annual income divided by 12. Earned income and pension income are counted at 50/100, and interest and dividend income under 소득세법 제14조제3항제6호 is not added if it is ₩10M or less.
What this means
A ₩2M-a-month National Pension counts as ₩1M of income, for an income premium of ₩71,900. What makes a retiree's bill grow is not income but the added property premium.

A married person qualifies only if both spouses meet the income test. Income follows Decree Art. 41(1), counting public pensions in full.
What this means
All three steps must be passed. 'Just stay under ₩20M' is only half right, and a single won of rental income shuts the door. A ₩2M-a-month National Pension (₩24M a year) fails the income test.

Frequently asked
I retired and my income fell. Why did my premium grow?
Because your status changed from employee to **regional member**. An employee is charged on salary alone with the employer paying half. A regional member pays it all and **a property premium is added**. On income alone the premium can even fall, because a ₩2M-a-month National Pension counts as only ₩1M of income, or ₩71,900. Add the ₩103,635 property premium on a home with a ₩220M tax base and the total is ₩175,535, more than the ₩107,850 share on a ₩3M salary.
If I stay under ₩20M a year, do I qualify as a dependent?
Only half right. There are **three layers** and you must pass all three: income of ₩20M or less, **no business income** (up to ₩5M tolerated if you have no business registration, and any housing rental income closes that exception), and a property tax base of ₩540M or less (₩900M if annual income is ₩10M or less). And the income test is applied to both spouses.
Does owning a car raise my health insurance?
No. Cars carried a premium for 35 years from 1989, but it was **abolished from the February 2024 bill**. However many cars you own, they have no bearing on health insurance.
I am an employee who owns a home. Does that raise my premium?
No. An employee's premium is charged on **monthly pay (salary) alone** and property is not counted. However, very large income outside salary triggers a separate additional premium.
What is voluntary continued membership?
A buffer that lets you keep paying, for a period after leaving a job, at roughly the level you paid as an employee. It is used when the regional premium would be larger. Eligibility and duration need **confirming with the Service**, and there is an application deadline you cannot recover once missed, so ask before you retire.
